1. US passport (Current and if PIO issued on an older passport, that passport copy as well is required)

2. If ever held Indian passport, Indian passport and Naturalization certificate 

3. Proof of address (Lease copy, driver's license, or utility bill)

4. Two passport size photographs   (2x2, white background).              

5. Parents' Passports and Marriage Certificate (if holding Indian passport, then proof of valid status)

6. Birth Certificate if born outside of India

7. PIO CARD

8. Applicant's signature or thumb (if below 5 years) on a white sheet of paper.
