2. PIO Card

3. Indian Passport and Renunciation Certificate

4. If married, Spouse's Passport (Indian & US if applicable, renunciation certificate, valid status proof), Spouse’s naturalization certificate, and marriage certificate 

5. Naturalization Certificate

6. Proof of address (Lease copy, driver's license, or utility bill)

7. Two Passport Size Photos (2x2, white background).

8. Applicant's signature on a white sheet of paper.

2. We shall complete the application forms and email you the documents checklist, courier label, and forms to ship to the New York office.
